Fall 2019 Sports Physicals
Physicals are available from Family Medicine at Graham Park on:

Aug 27th  6:30pm-8:30pm

The cost is $30, cash or check made payable to:  Family Medicine

All sports physicals must be filled out using the PWCS physical form! You can stop by school to pick one up.

Eligibility for Athletic Teams
In order to try out for any athletic team, including Cheer, students must meet these Academic and Conduct requirements:

Academic Eligibility – Students must be academically eligible to represent a school in any athletic activity.
Students receiving 2 or more “F’s” on a report card will be academically ineligible to participate in activities
the following 9 weeks. Academic eligibility will carry over from the 4th grading period to the 1st grading period
of the next school year. Rising 6th grade students start the school year with a “clean slate”, however the student
can be declared ineligible to participate in the 9 weeks following a marking period if criteria are not met.

Conduct Eligibility – A student is determined to be conduct ineligible as determined by 2 or more “U”s in Conduct
for the preceding 9-week marking period. The student is ineligible to participate in the following 9-weeks marking
period. Conduct eligibility will not carry over from the 4th grading period to the 1st grading period of the next school year.
 The year starts with a “clean slate”; however, the student is ineligible to participate in the 9 weeks following a marking period
 if criteria are not met. A conduct ineligible student may meet eligibility requirements at the interim and be eligible to try out
for a sport in the next season if the proper criterion is met. 

Attendance Eligibility - Students must be present ½ day to participate in or attend any after-school activities,
including athletics games, practices and tryouts.

Concussion Training Update from PWCS

Concussion Training is available for Fall middle school sports as follows:update
- Returning seventh and eighth grade students and parents may use the online training program to meet the concussion training requirement.
- Rising sixth grade students or new student to Prince William County are required to participate in the scheduled Face to Face presentations the school.

You must meet all requirements and be Academic and Conduct Eligible in order to try out for any sport!

2019-2020 Concussion Education Presentations

All middle and high school student athletes and their parent/guardian 
are required to complete an annual Concussion
Training. Since there are sufficient differences in the way concussions are handled at the middle school and high school
levels, parents having children in both middle and high schools are required to attend both a middle and a high school

Face-to-Face Training:

All rising 6th grade and 9th grade student athletes, as well as student athletes new to the county, are required to attend
a Face-to-Face training with a Parent/Guardian. For Middle School training sessions, see the Files Section at the
bottom of the page. You may attend any one of these sessions.

REMINDER: Parents and students, please make note of each school's Face-to-Face Training start times and arrive on
time to complete the entire session.
 Doors will be closed once the training begins. Late arriving parents and students will not be permitted into the
training once it has started.
